* HECM Counselor Roster Upload Instructions *
 
In order to complete processing for your request to be added to FHA's HECM Counselor Roster, you must provide an electronic version of the application (Application for Home Equity Conversion Mortgage (HECM) Counselor Roster, form HUD-92904) and each HECM training certificate. All scanned training certificate documents must be saved as Portable Document Format (PDF) files. Optionally, you may upload supporting documentation as well. Other scanned document files can be uploaded as a PDF or a Microsoft® Word® document (.DOC or .DOCX).
  1. Application
    The following five steps are required to complete the application:
  2. 1. Generate Click Generate Application Form to automatically generate form HUD-92904 with all information entered on the HECM Counselor Roster Application placed in the appropriate boxes on the form. No other information is required on the form.
    2. Print Using the Acrobat Reader Print button, print the generated form (should be two pages). After printing the form you can close the pop-up window containing the application form
    3. Sign Once the form has been printed, you must sign the form (box #12)
    4. Scan Using a scanner, scan both pages of the signed application into one PDF file. Don't scan each page into its own file since the system only permits you to upload the two pages of the scanned application as a single file.
    5. Attach On the HECM Counselor Roster Application - Upload Files page, use the Browse button for Upload Application to locate the scanned version of your application and attach it to the page to be uploaded.

  3. HECM Training Certificate
    An electronic copy of each HECM Training Certificate listed on your application must be provided. The certificate should include (at a minimum):
    1. Course Name
    2. Training Company Name
    3. Completion Date

    To attach each of the certificates to be uploaded, do the following:
  4. 1. Scan Scan each certificate, saving each as a separate PDF file.
    2. Attach On the HECM Counselor Roster Application - Upload Files page, use the first Browse button for the Certificate File to Upload to locate the scanned version of the first certificate and attach it to the page to be uploaded. Repeat this step for each certificate.
    3. Attach Supporting
        Documentation
        (optional)
    Use the Browse button for the Additional Documentation to Upload to locate the supplemental documentation file for the corresponding certificate and attach it to the page to be uploaded. Repeat this step as needed.

  5. Upload the Attached Files
  6. 1. Verify This is an important step: Verify that all the files to be uploaded are attached to the HECM Counselor Roster Application - Upload Files page.
    2. Upload Click Send to upload all attached files.
