- Application (optional if not already on file in electronic format)
The following five steps are optional if an electronic copy of your application is already on file:

| 1. Generate |
Click
Generate Application Form to automatically
generate form HUD-92904 with all information entered on the
HECM Roster Application placed in the appropriate boxes on the form.
No other information is required on the form.

| 2. Print |
Using the Acrobat Reader Print button, print the generated form (should be two pages). After printing the form you can close the pop-up window containing the application form |
| 3. Sign |
Once the form has been printed, you must sign the form (box #12) |
| 4. Scan |
Using a scanner, scan all pages of the signed application into one PDF file. Don't scan each page into its own file. |
| 5. Attach |
On the HECM Roster Renewal - Upload Files page, use
the Browse button for Upload Application to locate the scanned version of your application and attach it to the page to be uploaded. |

HECM Training Certificate
An electronic copy of each HECM certificate being added or renewed must be provided.
The certificate should include (at a minimum):

- Course Name
- Training Company Name
- Completion Date
To attach each of the certificates to be uploaded, do the following:

Scan each certificate, saving each as a separate PDF file. |
| 2. Attach |
On the HECM Roster Renewal - Upload Files page, use the first
Browse button for the Certificate File to Upload to locate the scanned version of the first certificate and attach it to the page to be uploaded. Repeat this step for each certificate. |
- Upload the Attached Files

| 1. Verify |
This is an important step: Verify that all the files to be uploaded are attached to the
HECM Roster Renewal - Upload Files page.

Click Send to upload all attached files. |
