Only authorized HUD personnel or the originator or sponsor/agent of the case can get the completed Conditional Commitment form issued by HUD. The procedure is explained below.

Getting the Conditional Commitment Form

  1. On the Conditional Commitment Request page, enter the case number assigned to the mortgage loan in the FHA Case Number field. Type the first three characters of the case number in the first box. Press Tab to go to the next box and type the remaining seven characters.

  2. Select the version of the form in the Form Type field.

  3. Click Send. If there are no errors, the Conditional Commitment form page appears.

    If there are errors, the Conditional Commitment Request page appears with a Message field above the FHA Case Number and an Error(s) Detected field below it. Review the information in these fields. Make any necessary corrections and process again.

  4. Use your Internet browser's print capability to print the form. To view the other version of the Conditional Commitment form, click the appropriate link in the upper right corner of the page: View Homebuyer's Copy or View Lender/Case Binder Copy.

Getting the Information Sheet

  1. To get the information sheet pertaining to the form, click Lender Information, Case Binder Information, or Homebuyer Information on the Conditional Commitment Request page or the Conditional Commitment form page.

  2. Use your Internet browser's print capability to print the information.
