Use the FHA Loan Underwriting and Transmittal Summary form (HUD-92900-LT) to obtain secondary financing information. Additional information (listed below) may be obtained, if applicable, in the secondary financing loan instruments included in the case binder:

FHA case number for the mortgage loan,

Name, address, and Tax ID for the nonprofit agency providing funding assistance,

Amount and purpose for the secondary financing provided to the borrower, and

Statement indicating whether the secondary financing:
 
will close in the name of the government entity, or
 
will be closed in the name of the nonprofit agency and held by the government entity.
