Origination Reports - Processing
There are two main parts to Origination Reports:

Requesting an Origination Report

Request an origination report as follows:

  1. Select Origination Reports Request on the Origination Reports menu page. The Origination Reports Request page appears.

  2. On the Origination Reports Request page, select the HUD Field Office location of the cases to be reported from the drop-down Field Office list.

Note: If necessary, view a list of the Field Offices by Field Office Codes.

  3. Select the report you are requesting from the drop-down list in the Report Type field. Only one report can be selected at a time.

  4. Enter the time range of the report by typing the date the reporting period starts in the Beginning Date field and the date the reporting period ends in the Ending Date field. Use an mm/dd/yyyy format, such as 04/15/2002. The number of days from the beginning date to the ending date cannot exceed 90. Only the most current 13 months of information is available.

  5. Specify the originating lenders to be included in the report. Type the first five characters of the requesting lender's 10-character FHA ID in the Home Office ID field. This produces a report of all associated originating lenders with FHA IDs that begin with the same five characters (branch offices). For example, if 99999 is entered, the report will include all originating lenders with FHA lender IDs that begin with those characters.

    -or-

Type the 10-character FHA ID for each originating lender to be included in the report in the boxes provided in the Originator IDs field. Up to ten originators can be entered. The first five characters of each FHA ID entered must be the same. This produces a report that includes only the specified originators. For example, if 9999900024, 9999900031, and 9999900076 are entered, the report will include only information related to those originators.

Note: An authorized HUD user can enter up to ten originators with completely dissimilar FHA IDs in the Originator IDs field.

  6. Type the name of the contact person for the requesting lender in the Last and First fields.

  7. Type the telephone number of the contact person for the requesting lender in the Contact Phone field.

  8. Click Send. If there are no errors, the Origination Reports Request Results page appears with a summary of the report request. A Message field at the top of the page states: Origination Reports Request Has Been Successfully Completed.

    -or-

If there is an error, one of the following occurs:

An error message box appears. Click OK to clear the message. Make the correction and process again.
The Origination Reports Request page appears with a Message field above the request criteria fields and an Error(s) Detected field below them. Review the information in these fields.
