Underwriter Registry - Field Descriptions

The field descriptions for the Underwriter Registry page are listed below in the order they appear on the page. (View field descriptions in alphabetical order.)

Field	Description
Select Function	List of available tasks, including:
	Add Underwriter: Enter information for a Direct Endorsement (DE) underwriter not previously registered with the FHA. Upon successful completion, an FHA-assigned identification number is issued.
	Change Underwriter: Update an existing DE underwriter's information listed with the FHA.
	Terminate Underwriter: Removes the underwriter from Active status.
Underwriter ID	Four-character FHA-assigned identification number issued to the underwriter.
Social Security No.	Social Security Number of the underwriter.
Underwriter Name	Last name, first name, and middle initial of the underwriter.
Former Name	Previous last name, first name, and middle initial of the underwriter if individual was registered under a different name.
Phone Number	Business telephone number of the underwriter.
Email Address	Business electronic mail address of the underwriter.
Confirm Email Address	Repeat of the underwriter's electronic mail address for verification.
Attention Line	Name of the company or individual to which correspondence is directed.
Hse No	House number of the underwriter's business mailing address.
Unit	Designation for a specific dwelling associated with the underwriter's business mailing address. Example: 22 B Main Street.
Pre	Street direction preceding the street name of the underwriter's business mailing address. Example: NW Greenlake Drive.
Street Name	Name or number of the street in the underwriter's business mailing address. Examples: Elm or 42nd.
Type	Street classification such as Blvd, Dr, or Pkwy in the underwriter's business mailing address.
Post	Street direction following the street name in the underwriter's business mailing address. Example: Greenlake Drive NW.
City	City of the underwriter's business mailing address.
St	State of the underwriter's business mailing address.
Zip Code	U.S. Postal Service Zonal Improvement Plan (Zip) code of the underwriter's business mailing address.
Current Employer	First five characters of the FHA's unique 10-character identifier for the lender's home office by which the underwriter is currently employed.
Hired	Start date (mm/dd/yy) of underwriter's employment with Current Employer.
Former Employer	First five characters of the FHA's unique 10-character identifier for the lender's home office by which the underwriter was previously employed.
Termination Date	Last date (mm/dd/yy) of underwriter's employment with Former Employer.
[Certification]	Yes/No (Y/N) input acknowledging statement that the lender has reviewed the underwriter's credentials and he/she meets or exceeds all DE underwriter requirements.
