Field Description

Application Type Indicator specifying whether the applicant is applying as an Application Coordinator or Standard User.
First Name First name of the applicant.
Middle Initial Middle initial of the applicant.
Last Name Surname of the applicant.
Social Security Number Social Security Number of the applicant.
Password Six-character password created by the applicant.
Re-enter Password Repeat of the six-character password entered in the Password field.
Phone Number Telephone number of the applicant.
Email Electronic mail address of the applicant.
Agency Name of the Federal agency (and program) participating in CAIVRS with which the lending institution is authorized to do business.
Lender ID Ten-character identifier issued to the lending institution by a Federal agency (and program) participating in CAIVRS. Note: For Veterans Affairs, this is the first 10 characters of the ID issued.
Organization Name Name of the lending institution that employs the applicant.
Mother's Maiden Name Surname of the applicant's mother before marriage.
