SSN Social Security Number of the 203k consultant. (SSN may be masked based on the user's authorizations.)

Other: Any reason not listed above for ending or temporarily suspending activity.
Impose Date Beginning effective date (mm/dd/yyyy) of the sanction.
Lift Date Ending date (mm/dd/yyyy) of the sanction.
Comments Additional information about the sanction.
Termination Information
Termination Date Date the 203k consultant was terminated and no longer has FHA approval.
