Field Description
Single Case
FHA Case NumberUnique 10-digit identifier assigned to the mortgage by the Federal Housing Administration (FHA).
OriginatorTen-digit FHA ID and name of the FHA-approved lender that originated the mortgage.

Sponsored Originator Nine-digit Employer Identification Number (EIN) of the mortgage banker or broker that originated the mortgage and was established as a sponsored originator by an FHA-approved lender.
Borrower Name Last name, first name, and middle initial of the borrower.
Property AddressHouse number, street name, city, state, and zip code of the property (including unit number, if applicable).
Reason for CancellationIndicator specifying the reason for the case cancellation.
CommentsReason for the case cancellation (if Reason for Cancellation is Other) or any additional remarks on the cancellation.
Action Indicator specifying whether the case was canc, ,eled or reinstated or the cancellation reason was changed. Note: If the case was reinstated or the cancellation reason was changed, the date the case was initially canceled is displayed and, for HUD users, the FHA Connection user ID and name of the user who canceled the case are also displayed.
Multiple Cases (HUD Personnel Only)
Upload complete; nn records processed Verification that the upload of the spreadsheet is complete and the number of records that were processed.
Case Number Unique 10-digit identifier assigned to the mortgage by the Federal Housing Administration (FHA).
Process Status Explanation of the outcome of the spreadsheet upload process for the case.
