Field

Description

FHA Case Number

Unique 10-digit identifier assigned to the mortgage by the Federal Housing Administration (FHA).

Originator

Ten-digit FHA ID and name of the FHA-approved lender that originated the mortgage.

-or-

Sponsored Originator

Nine-digit Employer Identification Number (EIN) of the mortgage banker or broker that originated the mortgage and was established as a sponsored originator by an FHA-approved lender.

Borrower Name

Last name, first name, and middle initial of the borrower.

Property Address

House number, street name, city, state, and zip code of the property (including unit number, if applicable).

Reason for Cancellation

Indicator specifying the reason for the case cancellation.

Comments

Reason for the case cancellation (if Reason for Cancellation is Other) or any additional remarks on the cancellation.

Action

Indicator specifying the action to be taken:

Cancel Case: Cancel the case and make it inactive for processing. Note: This option appears only if the case is still active and was not canceled.

Reinstate Case: Reinstate a canceled case and make it active again for processing. Note: This option appears only if the case was canceled. The date the case was initially canceled is displayed and, for HUD users, the FHA Connection user ID and name of the user who canceled the case are also displayed.
