Field Description

FHA Case Number Unique 10-digit identifier assigned to the mortgage by the Federal Housing Administration (FHA).

Primary Borrower Name Last name, first name, and middle initial of the primary mortgagor.

Property Address House number, street name, city, state, and zip code of the property (including unit number, if applicable).

Referral Date Date the lender provided the borrower with a referral list of FHA-approved housing counseling agencies that provide HECM counseling.

Referral Type Indicator specifying whether the lender provided the borrower with a list of FHA-approved housing counseling agencies that offer HECM counseling.

Not Referred

Not Referred Reason Reason the lender did not provide a list of FHA-approved housing counseling agencies that offer HECM counseling.

Confirm National Counseling Agencies Referrals List of the national intermediaries offering housing counseling via telephone to which the borrower was referred.

Lender Referred Counseling Agencies - Agency ID [1, 2, 3, 4, 5] List of the housing counseling agencies to which the borrower was referred that are in the local area and/or state in which the borrower resides. Five-digit identifier issued to the FHA-approved housing counseling agency by HUD's Housing Counseling System (HCS). The first digit is "8" or "9" followed by a unique four-digit number.
