MessageNotification that processing was successful.
FHA Case NumberUnique 10-digit identifier assigned to the mortgage by the Federal Housing Administration (FHA).
ADP CodeAutomated data processing code related to the section of the National Housing Act under which mortgage insurance was requested.
Amortization PlanAmortization plan code for cases with a Graduated Payment Mortgage (GPM) or Growing Equity Mortgage (GEM).
A: Annual increase of 2.5% for five years - Section 245(a) GPM.
B: Annual increase of 5.0% for five years - Section 245(a) GPM.
C: Annual increase of 7.5% for five years - Sections 245(a) and 245(b) GPM.
D: Annual increase of 2.0% for ten years - Section 245(a) GPM.
E: Annual increase of 3.0% for ten years - Section 245(a) GPM.
F: Annual increase of 4.9% for ten years - Section 245(b) GPM.
L: Annual increase of 1% for life of loan - GEM.
M: Annual increase of 2% for life of loan - GEM.
N: Annual increase of 3% for life of loan - GEM.
O: Annual increase of 4% for life of loan - GEM.
P: Annual increase of 5% for life of loan - GEM.

LTV RatioPercentage of the property value or sales price that the mortgage covers.
Borrower TypeClassification of the borrower in relation to the property.

Living UnitsNumber of separate dwellings. For example, a two-family house has 2 living units.
Control NumberUnique 10-character identifier that is used to track the specific FHA case number.
Name of Mortgagor (last, first, MI) Last name, first name, and middle initial of the borrower.
Mortgage AmountAmount of the principal obligation stated on the mortgage (or other security instrument); dollar value of the mortgage insurance to be issued for the property. Includes upfront mortgage insurance premium (UFMIP), if upfront MIP is financed. (Not applicable to HECM cases.)
Interest RateRate at which interest is charged on the mortgage.
Monthly Payment (P/I)Mortgage payment, including principal and interest, due from the borrower to the lender on a monthly basis. (For HECM cases, REVERSE appears instead of an amount.)
Name of Co-Mortgagor (last, first, MI)Last name, first name, and middle initial of the coborrower.
Maturity DateMonth and year (mm/yy) of the final mortgage payment.
First PaymentMonth and year (mm/yy) of borrower's first mortgage payment.
Endorsement DateMonth, day, and year (mm/dd/yy) the mortgage (case) was approved for FHA insurance.
Address of PropertyHouse number, street name, city, state, and zip code of the property (including unit number, if applicable).
Mortgagee Name, Address, and ID Number FHA ID and name of the lender that originated the mortgage loan, or its sponsor or agent; address and c/o line of the requesting lender.
