First Name First name of the applicant.

Middle Initial Middle initial of the applicant.

Last Name Surname of the applicant.

Social Security Number Social Security Number of the applicant.

Organization Name Name of the organization with which the applicant is associated.

Telephone Number Telephone number of the applicant.

E-mail Address Electronic mail address of the applicant.

Mother's Maiden Name Surname of the applicant's mother before marriage.

Title II Lender ID FHA's unique ten-character identifier assigned to the financial institution approved for participation in the mortgage insurance programs authorized by Title II of the National Housing Act.

Service Bureau FHA's unique five-character identifier assigned to the HUD-approved agency authorized to service a Title I and/or Title II lender's loans.

Applying for Title II Applications Title II applications on the FHA Connection for which the applicant requests access.

Title I Lender ID FHA's unique ten-character identifier assigned to the financial institution approved for participation in the mortgage insurance programs authorized by Title I of the National Housing Act.

Applying for Title I Applications Title I applications on the FHA Connection for which the applicant requests access.
